经口胆胰管镜的临床诊治进展

胆胰系统疾病是消化道诊治的一大难点,ERCP是胆胰系统疾病的常用诊治手段,广泛应用于临床。然而,ERCP无法对胆胰管疾病进行直视化诊治,无法精准评估病变的程度。随着科学技术的进步,经口胆胰管镜应运而生,使得消化道最后一个盲区得以点亮。经口胆胰管镜在临床诊疗的应用主要包括胆胰系统的直视下诊断和靶向活检、辅助射频消融和光动力治疗、困难胆胰管结石辅助碎石、胆囊疾病诊治、阑尾疾病诊治等。因此本文将围绕经口胆胰管镜的临床诊治进展进行综述。

Abstract:

Biliary and pancreatic diseases are a major difficulty in the diagnosis and treatment of digestive tract. ERCP is a common method for the diagnosis and treatment of biliary and pancreatic diseases and is widely used in clinical practice.However, ERCP cannot directly diagnose and treat biliary and pancreatic duct diseases, and cannot accurately evaluate the degree of lesions. With the progress of science and technology, the peroral biliary-pancreaticoscopy came into being, making the last blind spot of the digestive tract to be lit. The application of peroral biliary-pancreaticoscopy in clinical diagnosis and treatment mainly includes direct diagnosis and targeted biopsy of the biliary and pancreatic system, assisted radiofrequency ablation and photodynamic therapy, assisted lithotripsy of difficult biliary and pancreatic duct stones,diagnosis and treatment of gallbladder and appendix diseases.Therefore, this article will review the progress of clinical diagnosis and treatment of transoral cholangiopancreatic duct endoscopy.
