鼻胆引流管在困难鼻空肠营养管置管中的应用价值

Clinical application value of nasobiliary drainage tube in difficult nasojejunal nutrition tube placement

摘要:

为了评估常规鼻空肠营养管置管失败患者改用鼻胆引流管置管行肠内营养的有效性和安全性，2020年1月至2023年12月，石河子大学第一附属医院常规鼻空肠营养管置管失败后改用鼻胆引流管置管行肠内营养的全部32例病例纳入回顾性分析。结果显示，32例鼻胆引流管代替鼻空肠营养管置管均一次成功，操作时间（22.9±10.4）min（9~54 min），置管过程中及置管后患者均耐受良好，未发生鼻咽部损伤、穿孔、消化道出血等并发症，患者置管后营养状况得到明显改善。仅有1例术后2周内发生堵管，经导丝疏通后缓解，可以继续进行肠内营养。由此可见，对于常规鼻空肠营养管置管失败的患者，改用鼻胆引流管替代鼻空肠营养管行肠内营养安全、有效，值得进一步研究及推广应用。

Abstract:

To evaluate the efficacy and safety of nasobiliary drainage for enteral nutrition in patients who failed in conventional nasojejunal catheterization, a total of 32 patients in the First Affiliated Hospital of Shihezi University from January 2020 to December 2023 were included. All 32 cases of nasobiliary drainage tube placement after failure of nasojejunal nutrition tube placement were successful at the first attempt. The operation time was 22.9±10.4 min (9-54 min). All patients tolerated the catheterization process and post‑catheterization period well. No nasopharyngeal injury, perforation, gastrointestinal bleeding or other complications occurred during the operation. The nutritional status for patients was significantly improved after catheterization. Blockage occurred after operation in one patient within 2 weeks, and recovered after unblocking. Nasobiliary drainage tube is safe and effective for patients with conventional nasojejunal tube catheterization failure.
